However, not all payment information is included on the explanation of benefits. Contact us or call our payment vendor, VPay, at 855-808-8420. CareFactor does not have access to card information for card payments. VPay will have to be contacted.

If a provider needs to update information such as the tax ID number, payment address, or other information, an updated W-9 form must be submitted to us by fax at 614-766-1007, or mailed to our address on the contact page.
